When everything feels heavier than it should.
- Things you loved feel like chores
- Tired no matter how much you sleep
- Pulling away from friends
- Hard to start, hard to focus
- A heaviness you cannot explain
Depression is a medical condition, not a character flaw. SSRIs such as sertraline and escitalopram, and bupropion, are first-line treatments with decades of evidence. Finding the right one can take more than one try. That is normal, and it is exactly why follow-up matters more than the first prescription.

Week by week.
- Day 1A physician reviews your intake and history. If medication is appropriate, it ships to your door.
- Weeks 1 to 2Sleep and energy may shift first. Side effects, if any, are usually mildest after the second week.
- Weeks 4 to 8The window where mood usually lifts. If it has not, your physician adjusts the dose or changes the medication.
- Month 6 onwardGuidelines suggest staying on treatment for at least six months after you feel well, then tapering slowly with support.
Antidepressants carry an FDA boxed warning: they can increase suicidal thoughts in some people under 25, especially in the first weeks. Tell your physician about any change in mood straight away. If you are in crisis, call or text 988.

What if the first medication does not work?
It is common. In the largest real-world trial, about one in three people reached remission on their first medication, and more did after a change. Your physician expects to adjust.
Will it change my personality?
No. The goal is to feel like you again, not like someone else. If you feel flat or numb, tell your physician. That is a reason to adjust.
Is okay. right for bipolar disorder?
No. Bipolar disorder, psychosis and severe depression need in-person psychiatric care. We will tell you if okay. is not the right fit.
It is okay to ask. Start there.
